Please include a copy of the Purchase Order along with the form if you are a
government, civilian company or agency. This form includes instructions on how
to calculate your total amount for payment. After filling out the form, mail it
with payment to our address below:

payment. Otherwise, promptly mail payment to the above address. Don’t forget to fax
a copy of your purchase order
along with the completed order form.

Monday thru Thursday between the hours of 7:30am – 5:00pm EST and on
Fridays between 7:30am – 11:30am EST. If calling after store hours please
leave a message and include your name and phone number. Your call will be
returned as soon as we open the next business morning.
